Staying on medication may not avoid readmission
According to a new study conducted at Duke Medicine explains a targeted effort that would help the high risk heart failure patients that can stay on their medications. This will help to improve adherence to drug regimens but that had surprisingly very little effect lowering hospital readmission rates. It further suggests that the medication management is one of the many issues that patients are facing. If this issue is not solved properly it can get bitter.The co-author of the research, Bradi B. Granger, RN, Ph.D., director of the Heart Center Nursing Research Program at Duke explains, “We found that we could solve much of the problem around medication adherence, but that is not the whole issue," said. "This illuminates the opportunities we have to help patients in a much broader way, redesigning care to confront some of the issues that keep the most vulnerable patients from regaining their health.”
